Committed to providing high-quality, patient-centered care, we are currently seeking a motivated and skilled experienced Family Nurse Practitioner or family physician to join our dynamic team.

    Position Overview: We are seeking an experienced Family Nurse Practitioner or Family Physician to play a crucial role in our patient-centered care model. The selected candidate will collaborate closely with nursing staff to assess and address the medical needs of walk-in patients or those with same-day medical concerns. The Triage Clinician will not carry a primary care panel, focusing solely on triage duties and urgent or same-day medical needs to ensure prompt and efficient patient care.

    Responsibilities:

    • Collaborate with nursing staff to assess and prioritize walk-in patients of all ages and those with same-day medical needs.
    • Conduct thorough medical assessments and provide timely and appropriate medical care. Identify patients requiring a higher level of care, including those needing urgent transfer to an emergency room.
    • Communicate effectively with patients, ensuring a clear understanding of their medical concerns and treatment plans.
    • Consult with primary care providers and specialists as needed for further evaluation and management.
    • Utilize electronic health records (EHR) to document patient encounters, maintain accurate and timely records, and bill appropriately for visits.
    • Participate in regular meetings and training sessions to stay updated on best practices and protocols.

    Requirements:

    Qualifications:

    Board-certified Family Physician or Family Nurse Practitioner with a valid license to practice in the state of Massachusetts.

    Candidates should have a minimum of 3 to 5 years of experience caring for pediatric and adult patients.

    Previous experience in community health center setting is preferred.

    Strong communication and interpersonal skills, with the ability to work collaboratively with a diverse healthcare team.

    Proficient in the use of electronic health records (EHR) and other relevant technology.

    Commitment to providing compassionate and patient-centered care.

    Proficiency or fluency in Haitian Creole or Spanish preferred.
