    TheTeam Administrative Assistant, under the direction of the Clinical Manager, provides administrative support for all team operations within the program. Team Administrative Assistants at VNH act as the initial point of contact for inquiries regarding the team from patients, families, staff and other customers. This position facilitates and supports coordinated patient visits and staff schedules to achieve the highest level of team effectiveness. Team Administrative Assistants monitor compliance with plans of care, organizational policies and procedures, and regulations through regular chart audits and are also responsible for the consistent and timely compilation, management, and communication of team performance data.

    The Team Administrative Assistant

    • Works in tandem with clinical manager, clinical staff, and other team assistants to coordinate and improve patient care.

    • Provides basic administrative support to all team members and team functions.

    • Collaborates across the program and organization.

    • Triages calls, prioritizes based on urgency and complexity.

    • Acts as a liaison for patients and family members as needed.

    • Utilizes all data and personnel management software and tools accurately.

    • Confirms required visits are scheduled, when necessary, schedules (clinic, interval, supervisory, and welfare checks), reschedules, suspends, and cancels visits.

    Minimum Qualifications

    • High School diploma or equivalent; Associate’s degree preferred

    • Minimum of 2 years of experience in a healthcare setting

    • Scheduling experience strongly preferred
