    Description:

    St. Mary Medical Center, a member of Trinity Health Mid-Atlantic, is seeking a

    PRN Social Worker to join our Care Coordination team!

    Shift:

    PRN per department needs. Weekday and Weekend availability, required to work 2shifts/month, 1 holiday/yr.

    :

    The Social Worker provides direct service to patients and families and works with community agencies, physicians and hospital staff in order to provide psychosocial support and to facilitate discharge planning. Modifies approach based on age-specific needs of patient population to include child, adolescent, adult and older adult.

    Requirements

    • Bachelor's Degree in Social Work/Services

    • Master's Degree (preferred)

    • At least 1yr of Social Work experience
