    THE POSITION:

    Our Medical Care Coordination (MCC) program is on the cutting edge of providing comprehensive support services that empower patients to fully engage with their healthcare. As a Medical Care Manager (MCM), you’ll play a vital role in this mission, working closely with the MCC Program Manager and Director of Nursing. You’ll conduct assessments, develop integrated care plans, and monitor patient progress—ensuring that each individual receives the tailored support they need. While you won’t be providing clinical nursing services, your expertise will shine as you educate patients about treatment adherence, side effects, nutrition, and risk reduction.

    What You’ll Do:

    • Collaborate with our Men’s Health Foundation clinical and MCC teams to ensure patients have seamless access to high-quality medical care.
    • Conduct comprehensive assessments to identify each patient’s unique medical and psychosocial needs, addressing barriers to HIV treatment and adherence.
    • Provide brief interventions to boost treatment adherence and assist patients in managing medication side effects.
    • Create engaging health education materials on topics such as nutrition, co-infections, preventive care, and more for patients and their support networks.
    • Support your MCC team in delivering a positive and seamless patient care experience.
    • Facilitate patient referrals with MHF staff, ensuring access to essential services.
    • Coordinate with outside service providers to offer a comprehensive continuum of care.
    • Maintain accurate client files and documentation of services provided.
    • Prepare and submit timely reports as needed.
    • Participate in staff meetings, case conferences, and in-services to enhance team collaboration.
    • Manage transportation needs within the MCC program.
    • Connect patients with partner services like intimate partner violence support, medical services, and HIV testing.
    • Take on other duties as assigned to help our mission thrive.

    What You Need:

    • A valid RN license in California.
    • A minimum of three years experience in HIV clinic services, with a deep understanding of HIV treatment modalities and care guidelines.
    • Experience in chronic disease care/management is a plus.
    • Insight into the impact of psychosocial needs, trauma, and behavioral factors on health—along with the ability to intervene effectively.
    • Exceptional communication and organizational skills, both written and verbal.
    • Proficiency in Microsoft Office Suite (Word, Excel, Access, PowerPoint).
