The Palliative Care Advanced Practice Nurse (APN) provides leadership and oversight in the development and growth of the of the palliative care program. Provides direct and indirect clinical support to patients and families experiencing a chronic and/or terminal illness effecting quality of life. Works in the professional role as an expert clinician, consultant, leader, educator, and translator of research. The APN collaborates with an interdisciplinary team, the referring physician, and coordinates the care of patients and families. The APN ensures the palliative care program is in compliance with legal, regulatory, and clinical policies and procedures. Supplements and support the hospice service line as appropriate.
In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:
Provides independent advanced practice nursing care including but not limited to completions of assessment, diagnosis, outcome identification, planning, implementation and evaluation.
Performs comprehensive physical, psychological, social, spiritual, and cultural assessments.
Follows the National Consensus Project Clinical Practice Guidelines for Quality Palliative Care (NCP);Nursing: Scope and Standards of Hospice and Palliative Nursing Practicing (ANA), Hospice and Palliative Nursing: Scopes and Standards of Practice (HPNA).
Prescribes and/or recommends changes or enhancements to care plan including symptom management.
Actively participates and collaborates with the patient and family in the transitioning/discharge care planning.
Employs evidenced based, collaborative guidelines in designing and implementation of plan of care.
Completes comprehensive and descriptive documentation addressing the NCP guidelines including the wishes and goals of the patient and family.
Assures that clinical protocols, utilization review, case management, quality improvement, and compliance are implemented according to program policies and needs.
Assures compliance with Joint Commission and other regulatory standards.
Identifies opportunities for improvement in the structure and process of care delivery.
Completes patient documentation, submits bills for all patient visits and meets routinely with finance to review process and make changes as needed.
Participates in an organizational culture to support palliative care growth within and outside the system.
Participates in the development of short and long term strategic planning for program growth.
Establish workflow systems (e.g. flow of information, maintenance of records, delivery of care).
Develop and provide input for financial sustainability of the program.
Assist with establishing policies, procedures and processes for program operations.
Implement auditing process to monitor and ensure regulatory standards are being met.
Spokesperson for the palliative care program internally and externally.
Collaborates with leadership to enhance the palliative care program's community outreach to physicians, the public, healthcare facilities, and other applicable organizations in the community.
Participates in interdisciplinary team meetings to review patient and family cases.
Consults with health care team, referring physician.
Identifies self to colleagues for consultation surrounding clinical expertise, leadership, education, and research.
Acts as a consultant to the system and community.
Participates in the development and implementation of performance improvement projects.
Participates in bioethics committee.
Promotes advocacy, ethics, and systems thinking.
Consults with patient and families; identifies self as a resource and describes role.
Facilitates and coordinate patient and family conferences addressing anticipatory guidance.
Identifies learning needs of patient, family, staff, and system.
Educates the patient and family on disease process, prognosis and treatments available.
Participates in action plans addressing knowledge gaps surrounding palliative care within and outside the system.
Provides continuous education at all levels for associates and system.
Participates in the education of students and associates by serving as lecturer, preceptor, and/or mentor.
Educates patients and families regarding advance directives, palliative care and end of life decision making based on the wishes and goals.
Identifies evidence based practice gaps; promotes best practices.
Translates and integrates research findings into clinical practice.
Identifies, initiates and participates in clinical, quality improvement and change projects.
Disseminates research and clinical information through publications and/or local, regional, and national presentations.
Collects data regarding patient care, program. Disseminates outcome information for peer and system review.
May require daily travel.
May be required to be on call.
