    Work Schedule

    • Two 12 hour shifts per week (shifts begin at 4 a.m. and 7 a.m. - staff alternate shifts)

    • Every third weekend, occasional holiday;

    • 48hrs/2wks (0.6 FTE)

    Pay

    The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

    • $31.55 - $49.91 per hour

    A Brief Overview

    Accountable for providing care and treatment, within the scope of nursing practice, to patients on maintenance dialysis.

    What you will do

    • Assumes other job duties as per RN licensure, training and scope of practice when floating to other areas of the medical center r/t established floating guidelines.

    • Demonstrates critical thinking by synthesizing best practice protocols and standards with patient data, to decide on the best course of action.

    • Demonstrates ability to identify and respond to changes in the clinical condition, ad provide timely communication to appropriate care team member, using SBAR format.

    • Includes patient and significant others in plan of care including education needs from admission through discharge, evidenced by complete and accurate documentation, and use of appropriate interventions in accordance to hospital policies and evidenced based standards.

    • Actively participates in the learning experiences of orientees, co-workers, and students as assigned.

    • Takes ownership of professional development by staying current with professional practice standards; seeking learning opportunities in nursing specialty; and shares new learning (s), with peers formally and informally.

    • Engages in organizational initiatives and department operations by attending 50% of department meetings, 1 Employee Forum annually, 1 Mission for Staff annually, 1 volunteer experience annually, and other assigned in-services and meetings according to direction of department leader.

    • Consistently incorporates the nine guiding principles of safe medication administration, including the five rights of medication administration, and in compliance with Avera Medsafe - bar coding medication administration.

    • Participates in quality improvement practices by following best practice standards for Core Measures and incorporating Appropriate Care Interventions into patient plan of care, evidenced by: consistent, complete, and accurate documentation and achieving quality outcomes appropriate to the clinical condition and needs of the patient across the continuum of care.

    • Interacts professionally with physicians to coordinate care for patient across the continuum, not just the acute phase, of care.

    • Ensures readiness of area for anticipated, or unexpected emergency situations as evidenced by appropriate use of Rapid Response procedures, fulfilling assigned roles of code team, and appropriate follow through of safety policies.

    • Participates in the development of the trauma program for the hospital evidenced by consistency in the following of the program guidelines defined by MDH Level III trauma designation.

    • Actively participates in a committee or project improvement team that enhances the patient experience, improves efficiency of workflow, increases employee satisfaction, or positively impacts the mission of the organization

    • Systemically evaluates the quality and effectiveness of nursing practice. Evaluates his/her own nursing practice in relation to professional practice standards and relevant statutes and regulations. Acquires and maintains current knowledge in nursing practice. Completes all mandatory competencies. Contributes to the professional development of peers, colleagues and others. Utilizes research findings in practice if appropriate. Participates in department performance and process improvement and quality assurance activities, including promotion of dialysis access and referral for transplantation evaluation.

    • Maintains knowledge and proficiency in technical skills for: Dialysis runs; initiation and discontinuation from internal accesses and central lines; monitoring of vascular access function and appropriate reporting of signs of stenosis or occlusion; central venous catheter maintenance; intravenous infusions; medication administration; blood product administration; oxygen administration; and point of care testing. Performs cultures and LALs of machines and reverse osmosis systems as needed. Completes disinfection procedures and clean procedures effectively to meet requirements.

    • Complies with established departmental policies and procedures, scope of care, performance improvement activities, safety, environmental and infection control standards. Documents in the patient record according to departmental policy/procedure. Promotes and maintains documentation and an environment that is in compliance with state, federal and regulatory agencies, including CMS/ESRD-Renal Network, and Quality Assurance and Process Improvement efforts.

    • Coordinates care of patients in collaboration with Medical Director, Nephrologist, patients, families, other nursing staff and all ancillary departments.

    • Acts as a patient/family advocate. Collaborates with the patient, family, and health care providers in providing patient care. Decisions and actions on behalf of clients are determined in an ethical manner.

    • Identifies patient/family learning needs; Develops, implements and evaluates an educational plan for patients; Provides patient education related to but not limited to the dialysis process and procedures related to that process, including care and management of dialysis accesses.

    • Participates in or oversees collection, tracking and trending of accurate and complete vascular access data (including clotting events, fistula status and assessment), including monthly documentation in patient's chart of indications for catheter use, as assigned.

    • Perform all other duties as assigned.

    Essential Qualifications

    Required Education, License/Certification, or Work Experience:

    • Associate's Graduate from an accredited school of nursing or

    • Registered Nurse (RN) - Board of Nursing An active license in the state of practice Upon Hire

    Preferred Education, License/Certification, or Work Experience:

    • Bachelor's Graduate from an accredited school of nursing

    • 1-3 years Dialysis, Med/Surg, CCU or Emergency Unit
