    Description:

    HYBRID Schedule - Two days available to work from home and Three days required in office.

    Job Summary: Identifies specific performance improvement initiatives and works collaboratively with the local agencies/leaders in their development, implementation and evaluation of quality improvement programs. This position identifies, and collaboratively monitors the implementation of identified processes, to assure continued agency licensure, accreditation and coverage compliance requirements. Performs identified workflow review and completion to achieve standardization and organizational efficiencies. Works collaboratively with agency, assuring compliance with new regulations, standards in EMR documentation.

    Essential Functions

    • Identifies, collaboratively plans, implements and evaluates agency quality assurance/quality improvement/process improvement programs, focused on attainment of top decile outcomes, patient satisfaction and strategic aims.

    • Supports in-service education and other continuing education activities related to quality, outcomes, QA/PI. Monitors publicly reported outcomes and other data, utilizing quality software and EMR reports.

    • Assures agency staff compliance with federal/state and accreditation regulations through record review, case conferencing, mentoring and survey readiness/oversight functions.

    • Oversees Agency QA quarterly and annual reporting on QAPI, quality program initiatives.

    Minimum Qualifications

    • Bachelor's Degree required.

    • Four years' experience in homecare. Minimum 2 years in a supervisory, oversight role preferred.

    • Registered Nurse with current MI license

    • Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.

    • Experience serving on committees or other leadership activities

    • Must have well developed interpersonal and communication skills to provide effective interface with agency leaders, professional clinical staff and all other related team members.

    • Knowledge of quality improvement principles and related data collection and analysis

    • Must possess a valid Driver's License and have reliable transportation to and from the work site.

    • Must be able to articulate and demonstrate a commitment to the mission, vision and values of Trinity Home Health Services and to inspire active support of this in others.

    • Ability to consistently demonstrate commitment to the Business Code of Ethics and adhere to the Organizational Integrity/Compliance Program.

    • Must be able to adapt to frequently changing work parameters.

    • Proficient using computer programs, videoconferencing software, and telephone systems.

    • Ability to produce results and ensure accountability

    • May require some travel. Certification: OASIS Certification required within 6 months from date of hire. Must maintain recertification with no lapse in certification period Mental and Physical Requirements

    • Frequently lift, stands, and walks (35% of time)
