    Program Manager (RN) - Geriatrics

    Responsible for the clinical and operational leadership of geriatric programs and is the Nurses Improving Care for Healthsystem Elders (NICHE) coordinator. Reports to the Director of Nursing assuming leadership responsibility for the geriatric nursing practice related activities of patients in assigned areas, coordinates NICHE education activities, and oversees the geriatric resource nurse program. As the clinical and operational leader for nursing within Dartmouth-Hitchcock Medical Center’s Lebanon campus, partners with the Geriatric Center of Excellence (GCE) to assure superior nursing care of geriatric patients in an efficient and cost-effective manner that aligns with the elements of an age-friendly health system. Acts as primary contact for inpatient and outpatient departments, as well as internal, local and regional partners in coordinating care for geriatric patients.

    This position is primarily Monday through Friday but will require schedule flexibility as off hour needs arise

    Responsibilities

    • Partner with GCE to align nursing practice within the medical center with principles of an age-friendly health system

    • Assumes responsibility for assuring geriatric nursing care of patients across settings is age friendly and includes:

    • Assures nursing practice includes pertinent patients historical, physical, and laboratory data for intake assessments focuses on the 4 Ms of geriatric care.

    • Evidence-based planning that focuses patient goals.

    • Evaluation of care is based on patient goals and aligns with age-friendly health system metrics.

    • Ensures adequate supplies, equipment maintenance, and a safe care environment. Ensures regular checks on equipment and tracers for compliance. Rounds regularly with multi-disciplinary team as scheduled.

    • Coaches staff in service excellence, teamwork, communication, dependability, and professionals

    • Makes staffing and workload adjustments to meet the needs of our patients and customers.

    • Communicates regularly with the fellow managers and appropriate supervisors regarding safety concerns, patient and employee complaints, personnel issues, process changes and other relevant issues.

    • Partners with the department manager to develop key operating objectives and work to deploy action plans to achieve these objectives.

    • Provides all assigned personnel with regular performance feedback and ensures that all annual performance evaluations are written and provide clear feedback for performance improvement. Maintains performance management, corrective action and disciplinary records in support of policy.

    • Develops and monitors key performance measures or indicators for tracking operational performance and for monitoring progress on action plans. Formally reports progress and trend information to the director on a regular basis. Initiates cost saving opportunities.

    • Manages accurate and complete data for NICHE program.

    • Maintains records of the educational activities within the NICHE program including attendees, competencies, certifications and CEU’s awarded

    • Serves as a member of the GCE Leadership team to collaborate on mission, vision and planning.

    • Assumes responsibility for long-range planning of the NICHE Program. Maintain standards for Geriatric Emergency Room Program certification.

    • Develop unit based Geriatric Resource Nurses who serves as embedded experts and resources for best practice nursing care of older adults.

    • Participate in professional organizations as appropriate. Keep current in the field of geriatrics though journals, conferences, rounds and in-services.

    • Attend relevant GCE leadership and operational nursing leadership meetings as appropriate.

    • Collaborates with DH marketing and public relations to develop and implement GCE brand.

    • Develops and maintains educational materials for patients and staff regarding care of older adults as appropriate.

    • Collaborates with Workforce Development, Nursing Education and Office of Medical Education for internships and shadowing opportunities.

    • Perform other duties as required or assigned.

    Qualifications

    • Previous supervision experience preferred.

    • Registered Nurse with NH eligibility

    • Bachelors of Nursing degree with 5 years of nursing experience in Geriatrics or the equivalent of education and experience required.

    Required Licensure/Certifications

    • Registered Nurse with NH eligibility

    • BLS

    • Gerontological Nursing Certification (GERO-BC) within 1 year
