Position Overview:
We are seeking a compassionate and dedicated PRN Hospice/Palliative Care Nurse Practitioner to join our team. In this role, you will provide comprehensive palliative care and hospice services, focusing on symptom management, end-of-life care, and support for patients and their families. This position offers flexibility and the opportunity to make a meaningful impact in the lives of those facing serious illness.
Key Responsibilities:
Conduct comprehensive assessments of patients' physical, emotional, psychosocial, and spiritual needs.
Develop and implement individualized care plans in collaboration with interdisciplinary teams.
Provide symptom management and palliative interventions, ensuring comfort and quality of life.
Educate patients, families, and caregivers on disease progression, treatment options, and end-of-life care.
Coordinate with physicians, nurses, social workers, and other healthcare professionals to ensure integrated care.
Complete documentation in compliance with regulatory and organizational standards.
Participate in on-call rotations and provide telehealth support, as needed.
Qualifications:
Master's or Doctorate degree in Nursing.
Current Nurse Practitioner license in the State of Michigan.
Certification in Hospice and Palliative Nursing (CHPN) preferred
Minimum of 5 years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred
Strong communication, assessment, and critical-thinking skills
Ability to provide compassionate care in challenging situations
Valid driver's license and reliable transportation for field visits
