    Job Summary :

    The Post-Acute Coordinator is responsible for business development and promotion of RSFH post-acute services including Roper Rehabilitation Hospital, RSFH Home Health, RSFH Outpatient Rehabilitation and RSFH Hospice. The coordinator interfaces with the community as well as referred patients by coordinating the RSFH post-acute continuum of care. The coordinator is responsible for gathering patient demographic data, medical history and physician documentation and appropriately processing the referrals for admission to a post-acute setting in an expedited fashion. Professionally represents RSFH at conferences, community activities and through daily interactions. Performs business development calls with referral sources, tours clients and interested parties through the facilities as needed. The coordinator seeks opportunities to further develop the referral base and referral volume to RSFH post-acute services.

    Minimum Qualifications

    Education: Associates or baccalaureate degree required. Graduate from an accredited institution for either RN, LPN, PT, PTA, OT, OTA. SLP, Recreational Therapy or SW preferred.

    Experience: Minimum of two (2) years' experience in a clinical practice setting with experience in discharge planning, case management, and/ or intake/referral coordination. Previous sales/ marketing or business development experience is preferred. Hospice referral experience preferred.

    Licensure/Certification: Must be licensed or certified in the state of South Carolina for respective specialty. Must have current driver's license.

    Primary Source Verification (if applicable): SC Licensure in either RN, LPN, PT, PTA, OT, OTA, SLP or SW that can be primary source verified by SCLLR or Nursing- compact RN state licensing board or certification in recreational therapy. ~~~

    Knowledge/Skills: Must have exceptional customer service, communication and interpersonal skills. Demonstrates a thorough working knowledge of medical issues, post-acute federal regulations, and CMS/ commercial insurance requirements. Able to establish effective working relationships with staff, third party payers, referral sources, discharge planners, insurance case managers and physicians. Must have moderate to advanced computer skills. Ability to analyze data generated by all referral activities. Must function independently and have strong organizational, negotiation, and critical thinking skills.

    Contacts : All teammates of the RSFH post-acute continuum including physicians, discharge planners, administration, nursing, therapy, and medical records. Additional contacts include insurance companies, patient accounting, families/caregivers, other hospital departments, community facilities and the general public.

    Work Demands/ Environment :

    Frequent bending, stooping, standing, stretching, walking and sitting. Requires full range of body motion, including, handling and lifting patients, manual and finger dexterity, eye-hand coordination and safe driving skills. Occasionally lifts and carries items weighing up to 50 pounds. Requires frequent talking in person and on the telephone. Vision correctable to within normal limits. Specific visual requirements: visualization of documentation, inspection of patient, assessment of patients. Hearing required (hearing aid permitted). Requires fine motor coordination, operation of motor vehicle (or available transportation), operation of medical and office equipment or machinery. Occasional exposure to communicable diseases, body fluids, unclean environments, toxic substances, pharmaceutical preparations, hazardous and unsafe driving conditions, and other conditions common to delivery of health care in the community.
