    Position Title: Population Health Consultant (Direct to Employer)

    Position Summary:

    Provides analytic, programmatic, and project management support for the operations of Affinia Health Network. This role focuses on program development and project management that supports population and community health alignment, Affinia Health Network strategic initiatives, and care coordination for Affinia responsible lives. Assignments may require interfacing with, gathering information from, and making presentations to various levels of management and internal and external leaders.

    What the Population Health Consultant will need:

    • Bachelor's Degree in healthcare administration, business administration; or an equivalent combination of education experience in hospital operations, physician practices or customer support.

    • Valid State of Michigan driver's license.

    • Three years of experience in a health care setting. Experience managing several projects with associated deadlines. Experience with personal computers, information management, and computerized clinical systems.

    • Preferred Administrative Fellowship or Residency trained.

    • Working knowledge of basic ambulatory clinical processes of care and office practices; translating people, process and technology issues and conflicts.

    • Ability to work in a fast-paced information systems environment.

    What the Population Health Consultant will do:

    • Provides professional expertise and guidance on Affinia Health programs including Trinity Health Partners, Next Generation ACO, SIM and other key strategic initiatives.

    • Oversees and coordinates technical, clinical, and business program team members, assigns responsibilities, holds stakeholders accountable, and coordinates their work.

    • Oversees financial, strategic, and operational analyses to evaluate opportunities for new programs or project enhancements that align population and community health in the care of AHN responsible lives.

    • Serves as the Affinia Health Network liaison for internal and external initiatives in support of the strategic plan; assist with analysis as needed.

    • Coordinates advanced product knowledge and training on the system for staff. Educates clinicians, physicians and others on use of the various clinical / government and private payer driven initiatives that augment accomplishment of the Triple Aim. Works directly with ACO/CIN staff, as well as ACO/CIN participating physicians to ensure a strong knowledge base of the products and their use.

    • Facilitates implementation of new sites, services, or initiatives to strengthen Affinia Health Network by providing project management: develop and monitor work plans, conduct analyses, facilitate team meetings, and identify and address barriers.
