The Population Health Consultant provides analytic, programmatic, and project management support for the operations of Trinity Health Alliance of Michigan (THAM) . This role focuses on program development and project management that supports population and community health alignment, THAM strategic initiatives, and care coordination for THAM responsible lives. Assignments may require interfacing with, gathering information from, and making presentations to various levels of management and internal and external leaders.

This is a primarily remote position, with occasional in-person facilitation required for board meetings and strategic stakeholder engagement.

What the Population Health Consultant will need:
Bachelor's Degree in healthcare administration, business administration; or an equivalent combination of education experience in hospital operations, physician practices or customer support.
Valid State of Michigan driver's license.
Three years of experience in a health care setting. Experience managing several projects with associated deadlines. Experience with personal computers, information management, and computerized clinical systems.
Preferred Administrative Fellowship or Residency trained.
Working knowledge of basic ambulatory clinical processes of care and office practices; translating people, process and technology issues and conflicts.
Ability to work in a fast-paced information systems environment.
What the Population Health Consultant will do:
Provides professional expertise and guidance on THAM programs including Trinity Health Partners, Next Generation ACO, SIM and other key strategic initiatives.
Oversees and coordinates technical, clinical, and business program team members, assigns responsibilities, holds stakeholders accountable, and coordinates their work.
Oversees financial, strategic, and operational analyses to evaluate opportunities for new programs or project enhancements that align population and community health in the care of THAM responsible lives.
Serves as the THAM liaison for internal and external initiatives in support of the strategic plan; assist with analysis as needed.
Coordinates advanced product knowledge and training on the system for staff. Educates clinicians, physicians, and others on use of the various clinical / government and private payer driven initiatives that augment accomplishment of the Triple Aim. Works directly with ACO/CIN staff, as well as ACO/CIN participating physicians to ensure a strong knowledge base of the products and their use.
Facilitates implementation of new sites, services, or initiatives to strengthen Trinity Health Network by providing project management: develop and monitor work plans, conduct analyses, facilitate team meetings, and identify and address barriers.
Develops, executes and monitors process improvement plans to enhance care coordination, clinical outcomes, and operational efficiency of AHN.
Provides ongoing monitoring and assessment of Affinia Health Network program effectiveness by developing metrics and reporting processes, and analyzing results.
Oversees a variety of projects from conception through operational phases. Coordinate the assessment, analysis, planning, implementation and evaluation of complex/multi-faceted projects/ programs including providing data reports and analyses.
