Key Responsibilities:

• Direct Patient Care: Provide hands-on physical therapy services as outlined by our agency and certified by the patient’s physician.
• Comprehensive Assessments: Evaluate, document, and communicate patients’ progress and status to the physician and healthcare team.
• Education & Supervision: Teach and guide nurses, home health aides, family members, and patients to ensure a continuous and effective physical therapy program.
• Standards of Care: Deliver therapy services in line with accepted standards for each patient’s condition.
• Care Plan Development: Collaborate with the healthcare team and physician to develop personalized care plans.
• Detailed Documentation: Maintain accurate and timely clinical records for each patient, documenting limitations and progress.
• Therapeutic Treatments: Utilize therapeutic exercises, heat and cold modalities, muscle re-education, and biofeedback to treat patients.
• Diagnostic Testing: Conduct diagnostic and prognostic tests to assess muscle, nerve, joint, and functional abilities.
• Home Exercise Programs: Create and instruct patients and their families on home exercise programs.
• Timely Reporting: Complete initial evaluations, clinical visit reports, progress notes, and discharge summaries promptly.
• Supervision: Oversee and direct Licensed Physical Therapist Assistants (LPTAs) in executing the treatment plan.
• Interdisciplinary Collaboration: Participate in the Interdisciplinary Patient Conference Committee to ensure comprehensive patient care.
