    The VNA is seeking a full time physical therapist to work with our Indian River County Home Health division. Under the order of the physician, assumes primary responsibility for providing skilled physical therapy and health teaching to patients and their significant others in the home setting.

    Responsibilities:

    1. Administer treatments as prescribed by the physician.

    1. Observe patient's condition to monitor response to treatment.

    1. Counsel and teach patient and s/o to accomplish goals.

    1. Conduct physical assessment/histories to determine health problems.

    1. Establish and follow care plan which addresses problems requiring intervention.

    1. Formulate appropriate discharge plans.

    1. Meet with Clerical Therapy Coordinator on a regular basis.

    1. Maintain clinical records according to agency, state and federal guidelines.

    1. Assist in preparation of home health aides assignments and provide guidance to aides when applicable to physical therapy.

    1. Attends case conferences on an as needed basis.

    1. Participate in in-service educational programs.

    1. Provide supervision to licensed physical therapy assistants.

    Qualifications:

    • Possess current PT license in the State of Florida

    • Have a minimum of 1 year experience as a PT

    • Home Health Experience highly desired
