    Job Summary:

    The Physical Therapist Assistant (PTA) will provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist (PT). This role is responsible for implementing treatment plans, assisting patients in improving mobility and strength, and ensuring the highest level of patient care in their homes. The PTA will play a key role in supporting Nye Home Health Care's mission to enhance patient quality of life and independence.

    Key Responsibilities:
    • Implement physical therapy treatment plans developed by the Physical Therapist, tailored to meet patient goals and needs
    • Provide rehabilitative exercises, techniques, and modalities to help patients restore function, improve mobility, and prevent disability
    • Monitor and document patient progress, providing feedback to the supervising Physical Therapist and interdisciplinary team
    • Educate patients, families, and caregivers on techniques to improve patient outcomes and support therapy goals
    • Complete and maintain timely, accurate, and detailed clinical documentation in compliance with company policies and regulatory standards
    • Communicate effectively with team members, patients, and families to ensure coordinated care delivery
    • Adhere to infection control protocols and safety procedures during all patient interactions
    • Travel to patient homes within designated service areas to provide care
    Qualifications:
    • Graduate of an accredited Physical Therapist Assistant program
    • Current, active PTA license in the state of Nebraska
    • Minimum of 1 year of experience as a Physical Therapist Assistant; Home Health experience preferred
    • Current CPR certification
    • Valid driver's license and reliable transportation
    • Ability to work independently and manage a flexible schedule
    • Strong interpersonal, organizational, and communication skills
