Pharmacy Clinical Specialist - Internal Medicine
The Cleveland Clinic internal medicine services provide an exciting environment for high quality patient care for a high acuity patient population. Daily responsibilities include decentralized coverage of assigned units and teams, coordination of pharmaceutical care incorporating multidisciplinary involvement, therapeutic drug monitoring, minimization of safety issues, ensuring cost effective pharmacotherapy plans, and medication reconciliation. Collaboration with other healthcare professionals is imperative. We are recruiting a highly motivated pharmacist to provide patient care in our internal medicine and subspecialty areas, rotation between 2-3 teams. Other responsibilities include precepting APPE student and PGY1/PGY2 residents and participation in projects/ research work the supports the departments mission and representing pharmacy on appropriate committees.
Practice Model/Patient Population
Direct patient care responsibilities rotating between 2-3 of our internal medicine services or subspecialties.
There are 9 internal medicine teaching teams.
Subspecialties include hepatology, urology, and pulmonology.
Breakdown of decentralized service coverage and centralized pharmacy coverage: ~70% decentralized / ~30% centralized coverage.
Pharmacists work every 8th weekend rotating between decentralized internal medicine coverage (day) and centralized coverage within the main pharmacy (evening).
Other scheduling items: 8 weeks of evening shift coverage per year rotating between the decentralized internal medicine position and centralized coverage in the main pharmacy; assigned one holiday per year; apart of the night shift coverage rotation (approximately once every 18-24 months).
Cleveland Clinic's inpatient pharmacy areas support distributive and clinical pharmacy services provided by staff pharmacists, clinical pharmacy specialists and pharmacy technicians, supporting 60 nursing units, 80 operating rooms, 4 PACUs, and 10 cath labs, accounting for over 1,440 inpatient beds.
A caregiver in this position works 7:00am - 3:30pm or 1:00pm - 9:30pm.
Coordinate and provide pharmaceutical care for patients.
Participate in patient care rounds.
Perform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients.
Provide general and patient-specific drug information.
Educate clinical pharmacists, technicians, pharmacy students, post-graduate trainees, allied health professionals, nurses and physicians in pharmacotherapy.
PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy.
If hired for a specialty area where specialty residency training is available, then specialty residency is required.
Current state licensure as a Registered Pharmacist (RPh) OR eligible for reciprocation in the current state within 90 days of hire.
Completion of a post-graduate year 2 residency (PGY2) or fellowship OR completion of a post-graduate year 1 residency (PGY-1) and two years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmaceutical monitoring OR four years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmacotherapeutic monitoring.
For individuals required to administer immunizations: current Basic Life Support (BLS) certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire.
For individuals required to respond to adult or pediatric cardiopulmonary arrest events: current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the American Heart Association (AHA) or certification obtained within 1 year of hire.

Physical Requirements :
Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.
Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
Position often includes reaching, stretching, stooping, climbing steps and sitting.
Must have normal or corrected vision.
Must have ability to clearly communicate verbally by phone or in person
