    Job Description:

    The supervisor provides leadership to Patient Care Coordinators to support care management and interventions for patients and families. The supervisor provides clinical supervision and staff development. The supervisor will also assure that the work is accurate and in compliance with instructions and established procedures.

    Minimum or Preferred Qualifications:

    BS - Bachelor of Science nursing required. BS - Bachelor of Science Social Work or MS - Master of Science Social Work preferred. 2 years of acute care nursing experience required. Use of computer applications related to health care industry and Working knowledge of Microsoft Office Suite and Outlook required. 2 years of experience in any combination of: case management, home care and utilization review preferred. Knowledge of and experience with Interqual Criteria set and software systems preferred. 2 years of supervisory experience preferred. Registered Nurse upon hire required. Certified Case Manager (CCM) preferred. Master of Social Work (LMSW) preferred

    Salary Range:

    $64,786-$99,938 annually Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.
