    Job Summary and Qualifications

    DUTIES INCLUDE BUT ARE NOT LIMITED TO:

    • Responds to phone calls with a sense of urgency and, demonstrating good customer service skills
    • Gathers initial information required to open new case/call(s) and prepares clinical information/packet which is to provide to the clinicians for assessment and next steps in advancing patients care.
    • Receives and maintains EMTALA education, to prevent and or identify risk to facility and/or organization
    • Establishes and maintains a high degree of awareness and knowledge of facility medical staff, services and specialized programs.
    • Helps to locate available resources and coordinates information to support patient disposition and or admissions to appropriate unit/facility
    • Maintains active inventory of bed availability within the community for non-HCA bed placements and admissions
    • Responsible for accepting, tracking and facilitating hospital transfers and transports telephonically and via computer applications.
    • Provides updates to customers on status of patient case(s), i.e. assessment status and bed placement requests
    • Assists to schedule and prioritize discharge transports and supports the Patient Logistics Center by assuring rapid response to requests for services ( where applicable)
    • Enters and supports data entry for accurate monitoring and reporting on all key indicators related to calls, admissions and transports.
    • Ensures patient rights and confidentiality are maintained.
    • Participates as an active and collaborative member of the multidisciplinary team.
    • Deals effectively with dissatisfied customers and escalates cases to leadership for follow up
    • Engages actively as a collaborative member of a high-performing team environment.

    KNOWLEDGE, SKILLS AND ABILITIES:

    • Possess good communication skills, verbal and written.
    • Possess good organization skills and able to act on multiple tasks.
    • Demonstrates ability to prioritize.
    • Ability to self-manage by taking responsibility to initiate and take action.
    • Ability to follow established policy and procedures when performing job functions.
    • Ability to demonstrate good judgment and use critical thinking skills.
    • Proficient and understands medical terminology
    • Knowledge of basic hospital and transportation services with markets the PLC is serving.
    • Interacts and communicates with others in a way that promotes a positive and cooperative work environment.
    • Ability to comfortably use multiple computer-based programs and applications, often simultaneously.
    • Ability to channel concerns appropriately and to function calmly in stressful situations.

    EDUCATION:

    • Associate degree or higher preferred

    EXPERIENCE:

    • Prefer experience in a healthcare setting. (Acute Hospital, Outpatient Clinic or Practice etc.)
    • Prefer experience in Behavioral Health (example Behavioral Health Tech)

    Scheduling Requirements:

    • 12 Hour Shifts - Required
    • Flexibility in scheduling - Required
    • Rotating Weekends - Required
    • Holiday Shifts – Required

    Must live within 60 miles of a Patient Logistics Center Facility -PLC Locations include; Madison, TN 37225, Richmond, VA 23225, Jacksonville, FL 32216, Tamarac, FL 33321, Plano, TX 75075 or Denver, CO 80237- Required
