    Description:

    Come join our Oncology team at Saint Francis Hospital, a member of Trinity Health Of New England!

    Listed as based at Saint Francis Hospital but also responsible for patients based out of Johnson Memorial Hospital

    Position Purpose

    Assesses financial needs of new and established oncology patients related to clinical care, according to specified protocol. Provides services in areas of financial need to patients, families, and the health care team as appropriate. Provides patient and health care team education, links to resources and data collection in accordance with the prices identified. Provides these services throughout the continuum of care.

    What you will do

    • Assess the financial needs of THOfNE oncology patients, establish a positive supportive relationship, assist in developing a plan of care and thoroughly document the finding, recommendations and interventions made

    • Collaborate with the treatment team the financial implications of medical intervention on both the patient and the health care provider

    • Educate patients regarding insurance optimization opportunities specifically for patients enrolled in Marketplace policies, Medicare and Medicare part D

    • Educate qualifying patients regarding entitlement programs such as SSI, SSDI, Medicare Savings Program, Low Income Subsidy, and Medicaid

    • Enroll all qualifying Patients into patient Assistance Programs and Co-Pay Assistance Programs

    • Interact with Medicare, insurance companies, pharmaceutical companies and other assistance programs as a champion for the patients

    Minimum Qualifications

    • Bachelor's degree in finance, business, counseling, social work, or other healthcare related field required. In the absence of a Bachelor's will consider a combination of education and experience - minimum of 6 years of financial navigation experience

    • Minimum Two (2) years in medical field working with financial needs of patients

    • Experience with Medicare health plans, COBRA regulations, Medicaid guidelines, PAP and co-pay assistance programs

    • Licensed in the state of CT as a Clinical Social Worker if area of specialty
