We're hiring for a Nursing Supervisor for the Heart Failure Clinic in Meridian!

    Heart Failure is a small specialty clinic with the goal of educating patients about heart failure and managing their symptoms to keep them out of the ER. The clinic coordinates across various specialties as needed, including home health, palliative care, and hospice. The clinic also partners with Intermountain Medical Center in Utah to provide advanced heart failure therapies, such as transplant and left ventricular assist device (LVAD) surgeries. The Nursing Supervisor will also oversee the care transition team which consists RNs and a social worker who can assist with issues related to obtaining care, healthy diet, and organizing medications. In addition to administrative duties, the supervisor will also spend some time working alongside the staff and assists with Staff RN duties.

    What You Can Expect:

    • Supervise and oversee development of HF clinical staff and Care Transition team. Serve as a resource to answer their questions and ensure all work is completed in timely fashion.
    • Onboarding and training of new staff
    • Acts as a daily resource to the PFA, palliative provider, and pharmacist for their work in the clinic.
    • Work with Care Coordinator to review incoming referrals to assess appropriateness and assist with timely scheduling of hospital f/u patients.
    • Work with clinic manager, front desk, and providers to address scheduling cadence and access issues.
    • Keep clinic manager informed of what's happening in the clinic and work together to resolve any issues.
    • Manage staff schedule, PTO requests, etc. Ensure clinic is sufficiently staffed each day.
    • Work on Heart Failure initiatives and other special projects.

    Schedule:

    Hours are generally 7:00 to 5:30 4 days per week, with variable day off.

    Qualifications:

    • Education: Bachelor's of Nursing Degree
    • Experience: 3 years' relevant experience
    • Licenses/Certifications: Current RN licensure from the State of Practice and Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
