Nurse Case Manager (RN) Responsibilities:

• As a nurse case manager (RN), you will be responsible for the managing resident cases.
• You will evaluate resident's medical history, needs, and support network.
• You will chart and maintain documentation for residents.
• You will design resident treatment plans.
• You will develop and maintain a positive working rapport with all team members.

Requirements:

Nurse Case Manager (RN) Qualifications:

• Registered Nurse in the State of Illinois.
• No history of abuse or neglect
• Current CPR certification.
• At least three years experience in long term care.
• A positive communication style
