    Full Time (80 Hours Biweekly)

    Shift: Day (Four 10-hour shifts per week, rotating start time of 9:00 a.m. and11:00 a.m.)

    Every third weekend and every third Holiday required

    The emergency department (ED) is a fast-paced environment where nursing staff care for patients of all ages with emergency medical complaints. The high volume GSH ED sees about 45,000 patients each year. Our highly trained staff are supported by a Director, Nurse Manager, Assistant Nurse Manager, Clinical Coordinators, Clinical Educator, and On-Boarding Specialists. The ED specific orientation is comprehensive to include care for high risk patients, life threatening conditions and illnesses, as well as care of patients with psychiatric disorders, and lower acuity patients. Certifications covered in orientation are BLS, ACLS, and PALS.     General Summary

    Interviews patients and reviews medication-related documentation for the purpose of compiling a medication history for patients being admitted to the Hospital. Compiled history information is entered into the patient's electronic health record for the purpose of facilitating medication reconciliation by the admitting provider. May perform a variety of functions which impact the medication reconciliation process and patient care.

    Essential Functions:
    • Collects an accurate best possible medication list of the patient's home medications in anticipation of hospital admission.
    • Gathers information for best possible medication list from various resources (patient interviews, physician offices, community pharmacies, historical encounters, extended care facilities, or other acute care facilities) to ensure a complete and accurate entry into the hospital's electronic health record.
    • Effectively communicates with other health care professionals to prioritize patient care and identify patient specific needs.
    • Accurately enters the home medication list into the Hospital's information system for utilization by the provider responsible for admitting the patient.
    • Updates the medication list in a timely manner so it can be used as the foundation for the patient's admission medication orders.
    • Directs any unresolved issues to the admitting provider.
    • Gathers medication information for patients in the ED and those being directly admitted to the floor.
    • Greets, directs and assists patients, visitors, physicians and ancillary department staff.
    • Answers telephones using personable etiquette. Answers inquiries by providing accurate information or forwarding callers to the appropriate resource. Identifies emergency messages, directs information to the appropriate personnel and maintains confidentiality.
    • Communicates delays/difficulties in reconciliation process with appropriate disciplines.
    • Assists with care and maintenance of department equipment, hardware and supplies.

    • Demonstrates behavior consistent with System mission, vision, goals, objectives and patient care philosophy.
    • Maintains effective communication and good relationships with other discipline within the facility for the patient's benefit.
    • Treats all individuals with respect, courtesy and empathy. Willingly assists others with questions. Introduces self to visitors and offers assistance. Verbal and non-verbal responses are courteous, friendly and caring.
    • Participates in educational programs and in-service meetings.
    • Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

    Minimum Education:
    • High School Diploma or GED Required
    Work Experience:
    • Less than 1 year 3 - 6 months Required
    • Experience with medication names, their uses, doses and routes of administration. Preferred
    Courses and Training:
    • 4 weeks hands-on training in the department. Upon Hire Required
    • Medical Terminology. Upon Hire Preferred
