Maternity Care Management Care Navigator

Job Description:

The Maternity Care Management Care Navigator responsible for supporting the care management team by helping research and navigate members to the appropriate medical, behavioral health or community resource.

Position Details:

Under the direction of the care manager, the incumbent conducts referral management of cases, care management pre-screening, patient scheduling, patient consultation, and transition management, to facilitate effective and efficient operations of the care management and utilization management team.

This position provides operational support and transition duties, as assigned, for care management services for identified highest risk (complex) SelectHealth members by working collaboratively with the care management team, patients, family caregivers, significant others, healthcare providers, payers, community-based providers, and other involved parties to provide effective, efficient, and patient-centered care management services.

This role will work Monday-Friday during regular business hours. This is a hybrid position with in-office requirements.

Minimum Qualifications:

• Experience working successfully working in a remote environment or using Advanced Microsoft Suite, including Teams (chat, whiteboard, task tracking) & Outlook; Capable of troubleshooting internet connection, basic computer hardware set-up, ability to customize computer settings and use multiple monitors.

• Demonstrated experience and proficiency working in patient care, care management, transition / discharge planning, medical assistance, healthcare coaching, or patient care coordination

• Demonstrated excellent interpersonal and communication skills.

• Knowledge of medical issues

• Ability to represent the department with other agencies

• Demonstrated problem solving skills

• Ability to adapt quickly as needs arise.

• Knowledge of available health resources.

Preferred Qualifications:

• Maternity experience

• Care Navigator experience

• Spanish Speaking

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$21.98 - $33.48
