    Job Description:

    Responsible for coordination of care at designated site, inclusive of care path facilitation, data collection and application of criteria to identify and manage general and high-risk patients. Facilitates interventions, staff assignments and coordination issues utilizing system wide criteria as tools. Must possess working knowledge of managed care, inpatient, outpatient, and the home health continuum, as well as utilization management and case management. Incumbent must maintain their knowledge of leading practices in clinical care and payor requirements. Incumbent must maintain a working knowledge of the concepts associated with Performance Improvement.

    Minimum or Preferred Qualifications:

    BS - Bachelor of Science in Nursing or health related profession (i.e. social work) required. MA - Masters in Nursing or health related profession (i.e. social work) preferred. 3-5 years in Utilization, Discharge Planning, and Case Management required. Previous experience in applying utilization and care management principles is required. Advanced communication and inter-personnel skills. Excellent judgement and assessment skills. Clinical Social Worker (LCSW) within 1 year required.

    $73,763-$113,133 annually Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.
