    Description:

    The primary purpose of the Enrollment Specialist position is to serve as the contact for newly qualified referrals,

    providing overview of PACE model and obtaining initial information relating to clinical and financial eligibility. The

    Enrollment Specialist is required to promptly follow-up on all qualified referrals assigned to them in the PO's

    Customer Relationship Management Platform as well as accurately document activity in the Electronic Health Record.

    Works with the enrollment team to manage qualified referrals through the entire enrollment process. Promotes the

    PACE Program to referrals, family/primary contacts, and community agencies the potential referral may be associated

    with.

    Shift: Typical work schedule is Monday to Friday, 8 am to 5 pm with occasional evening or weekend hours required.

    Position Purpose: As an Enrollment Specialist at Trinity Health PACE, you will be the frontline representative, connecting with potential participants to guide them through the enrollment process. Your primary goal is to ensure eligible individuals receive comprehensive information about our program and seamlessly transition into our care.

    What You Will Do:

    • Serve as the initial point of contact for newly qualified referrals, providing an overview of the PACE model and assessing eligibility.

    • Follow up promptly on qualified referrals, documenting activities accurately in our systems.

    • Collaborate with the enrollment team to manage referrals through the entire enrollment process.

    • Promote the PACE program to potential referrals, their families, and community agencies through presentations and events.

    • Coordinate enrollment assessments, maintain documentation for tracking and reporting purposes, and assist with Medicaid application processes.

    • Communicate regularly with interdisciplinary team members and referral sources to ensure a smooth enrollment process.

    Minimum Qualifications:

    • Associate's Degree preferred, with at least two years of sales or marketing experience.

    • Valid driver's license, proof of car insurance, and reliable transportation required.

    • Demonstrated ability to communicate effectively, both verbally and in writing.

    • Proficiency with Microsoft Office suite and other necessary software.

    • Strong organizational skills and the ability to prioritize workload effectively.
