Summary:

The Licensed Clinical Social Worker is a key member of the Rush University System for Health community. This position provides in-person and virtual social work services for complex issues from a variety of referral sources and connects those patients and community members to relevant resources both internal to RUSH and in the community. This position supports relevant programming focused on supporting individuals experiencing housing insecurity or those who are unhoused also and may serve as a representative of their department in meetings and presentations. This position exemplifies the RUSH mission, vision and values and acts in accordance with RUSH policies and procedures.

Responsibilities:

Develops a comprehensive plan of care for the patient and/or family

• Works directly with patients and their families across the age spectrum to address complex issues that arise from unmet needs and provide the necessary social work services them achieve their care goals.

• Connects patients and their families to relevant resources, both at Rush and in the community, to address their needs.

• Provides care management and/or support programming for aforementioned population as outlined by Manager.

• Documents all client interactions in either the Epic and/or other spaces as indicated by Manager.

• Maintains relationships with referring Rush clinics and community partners through consistent and timely communication.

• Promotes Rush University System for Health clinical services by referring clients to relevant Rush clinics and community partners.

• Leverages experience to support team members and patients / community members in training, crisis management, etc. as needed.

• Represents the department at meetings, conferences or other public engagements on an as-needed basis.

Other information:

Required Job Qualifications:

• Master's Degree in Social Work.

• Licensed Social Worker (LCSW) in State of Illinois.

• Two years' experience in healthcare, care management, mental health and/or social services.

Preferred Job Qualifications:

• Experience with relevant population(s).

• Professional qualities such as initiative, self-reflection, commitment, responsiveness and adaptability.

• Knowledge of care management, community-based interventions, assessment, counseling and/or motivational interviewing.

• Computer literate (Microsoft Office and others).
