    Title: Hudson House Program Supervisor

    Reports to: Clinical Director of Operations

    Job Objectives:

    • Assume necessary duties of Hudson House (HH).
    • Provide direct supervision of the Hudson House Case Managers, Hudson House Administrative Assistant, and Hudson House van driver(s).
    • Work as integrated team member at HH in all functions of the HH.
    • Perform duties while actively promoting an environment consistent with MHA's Cultural Equity Policy and WELCOME Orange.

    Hudson House Program

    Responsibilities:

    • Manage the daily activities/operations of HH program.
    • Direct supervision of Hudson House Case Managers, Hudson House Administrative Assistant, and Hudson House van driver(s).
    • Develop a community education program to bring service providers into Hudson House on a regular basis for education on community resources and self-efficacy tools.
    • Complete all intake to admission functions in conjunction with clinical staff including Receiving referrals and informational phone calls, coordinating application process on prospective participants, gathering pertinent information from other providers (i.e. physicians, home care agency, social workers) scheduling and facilitating program tours, assessment meetings and scheduling admission days.
    • Maintain all Hudson House forms such as:
    • Transportation logs
    • Sign in / Sign out sheets for meals
    • Attendance Records
    • Food Pantry Inventory and Distribution Tracking.
    • Ensure professional relationships and teamwork with all MHA staff.
    • Write and articulate clearly and effectively including plans, reports and all technical information.
    • Oversight of staff's documentation of Individualized Recovery Plan (IRP) progress notes and program log sheets.
    • Oversight and monitoring of staff's ADP and time off requests.
    • Supervision and facilitation of groups and activities.
    • Oversight of building maintenance.
    • Driving HH Van as needed.
    • Conducting HH staff and member meetings.
    • Oversight of Employment and Training Administration Program.
    • Provide individual service planning and rehabilitation counseling with enrolled clients.
    • Assume necessary job responsibilities of other staff as needed.
    • Transport members to travel activities.
    • Facilitate current and relevant groups and travel activities according to member feedback and needs.
    • Manage caseload to maximize units of service.
    • Provide SPCM with all required monthly Scorecards, data collection and statistical/program reports
    • Provide data entry into NIMRS (NYS Incident Management and Reporting System) and Foothold Technology.
    • Perform 1 to 2 hours of weekly member and community outreach as needed.

    Other Duties:

    • Participate in MHA's committees as needed.

    Requirements:

    Qualifications:

    • Bachelor's Degree in related field with 2 years of relevant experience working with individuals with Serious Mental Illness and/or Substance Use Disorder

    OR

    Associate degree in related field with 4 years of relevant experience working with individuals with Serious Mental Illness and/or Substance Use Disorder

    OR

    High School Diploma or equivalent with 6 years of relevant experience working with individuals with Serious Mental Illness and/or Substance Use Disorder.

    • Bilingual – English / Spanish preferred.
    • Supervisory experience preferred.
    • Ability to work independently.
    • Effective written and oral communication skills.
    • Must be organized, self-motivated and have the ability to coordinate multiple tasks simultaneously.
    • Ability to exercise sound judgment under crisis situations and to abide by regulations regarding confidentiality.
    • Valid Driver's License and ability to drive with current driving record acceptable for agency insurance coverage.

    Terms of Employment/Benefits:

    • Exempt.
    • Forty (40) hours per week
