Interim HealthCare, a leading Home Health agency in South Carolina and 7X Top Workplace winner, has an immediate opening for a PRN Hospice Nurse Practitioner (NP) to be responsible for collaborating with the interdisciplinary team to ensure care coordination and patient centered care for terminally ill patients and their families.

What you'll do as a Hospice Nurse Practitioner (NP) - PRN:

• Provide face to face encounters for identified patients required for admission certification and recertification within the regulatory timeframes.
• Performs history and physical assessment to address the appropriateness of admission, medications required and self-administration status, and identification of special conditions and/or care required.
• Collaborate with designated registered nurse coordinating visits to ensure timely and efficient services.
• Collaborates with referring physician concerning assessment findings, diagnostic and therapeutic measures.
• Ensures regulatory compliance of all clinical practices with all federal and state regulations governing hospice agencies, organizational policies, and billing compliance within the scope of practice of the nurse practitioner.
• Prescribes medication and therapeutic measures for pain and symptom management, within bounds of the Nurse Practitioner Act. Experience prescribing Schedule II medications (preferred).
