    We are currently seeking an experienced:

    HOSPICE MEDICAL RECORD REVIEWER - RN

    Full Time, Days

    Job Summary:

    • Ensure review for complete and accurate ICD-10 coding of hospice records following coding guidelines for assigned chart reviews
    • Collaborate as directed by Reimbursement/Appeals Department with Staff Development when trends are identified and additional education needs are identified.
    • Demonstrates knowledge and understanding of audit tools and utilizes these tools daily for complete and accurate auditing of all assigned medical record reviews.
    • Demonstrates an understanding of applicable regulations, accreditation regulations, licensure, Conditions of Participation regulations and Liberty Homecare and Hospice established policies.
    • Applies knowledge of clinical standards of care for all disciplines as the medical record is coded/audited to ensure evidence based care is being planned and delivered.
    • Notify offices/clinicians of any deficient findings via email and follow up timely to correct deficiencies/errors.
    • If offices/clinician do not respond timely, notify HH Audit Appeals and Documentation Compliance Lead Nurse.
    • Keep Clinical Managers, Administrators, Clinical Director, Reimbursement/Appeals Director, Vice President and President informed of educational needs identified as part of the auditing/coding process.

    Job Requirements:

    • Associate Degree in Nursing with 3 years of hospice care and experience in medical coding for home health and hospice preferred.
    • Must possess strong computer skills to include working knowledge of commonly used software programs such as Microsoft Word, Excel, etc.
    • Requires strong interpersonal skills to work effectively and professionally with clinical staff and branch management staff daily.
    • Must be able to prioritize work assignments and be flexible to change assignments per Supervisor’s direction with minimal notice. Must be able to complete work assignments within a specified timeframe.
    • Must have the ability to objectively and subjectively translate clinical information into statistical data.
    • Requires in-depth working knowledge of hospice regulations to include licensure, conditions of participation and accreditation standards as well as regulations related to reimbursement per payer source and ICD-10 coding.
    • Must be able to consistently exercise intellectual judgment regarding clinical practice as reflected in documentation within medical record.
    • Must possess strong critical thinking and communication skills (oral and written) and be literate in the English language. Must be able to write and/or type.
