We are seeking a Home Health Speech Language Case Manager for our team to ensure that we continue to provide all patients with high quality, efficient care. The Primary Speech Language Pathologist in Home Health manages an assigned caseload of patients, providing therapy and clinical case management, taking responsibility to improve patient outcomes and to manage resources and assure cost effective, clinically effective, and coordinated services. Completes comprehensive assessment/examination/evaluation, develops/modifies plan of care in conjunction with patient/caregiver and physician, provides treatment visits, and provides discharge planning to these patients to facilitate patient/caregiver engagement for achievement of patient goals, clinical outcomes and positive patient experience. The Primary Speech Pathologist provides the majority of patient’s speech language therapy services and communicates handoff information/coordinates with other therapists/members of care team to support patient’s care throughout home health and the continuum of care.

Required Education: Master Degree in Speech Language Pathology

Required License: Current North Carolina license to practice Speech Therapy; ASHA certification; Certificate of Clinical Competence (CCC-SLP) or CFY; BLS/CPR Certification. Driver’s license required. 

Required Experience: One to two years related experience. 

Preferred Experience: Clinical affiliations having a wide variety of acute care procedures, especially video fluoroscopic swallow evaluations and non-instrumental swallow evaluations. Experience with cognitive and language disorders, motor-speech disorders, dysphagia treatment, especially with neurology, cardiopulmonary, head/neck cancer, traumatic brain injury, and tracheostomy and ventilation management. Certification preferred, including but not limited to, Vital Stim, NMES, LSVT, and Dysphagia Therapy.
