Job Summary

As a Health Coach Nurse, will help patient console and care. Inspire patients to engage in their health. Provide tools for healthy alternatives and habits.

What You'll Do

As a Health Coach Registered Nurse,

• You will assess client recommendations for patient populations.

• You will evaluate health coach interventions and respond to medical referrals. Your goal is to identify patients meeting care criteria.

• You will partner with teams and partners.

• You will involve patients in care and execute personalized care plans.

• Help identify and address barriers to accessing services.

• You will use communication skills to interact with patients in person, by phone, or digitally.

• Ensure patients receive necessary services through reminders, medication reconciliation, and reassessments.

What Will Make You Successful In The Role

• To succeed as a Health Coach RN, you need knowledge of nursing guidelines and patient care.

• Interpersonal, communication, problem-solving, and critical thinking skills are crucial.

• Familiarity with basic computer skills is also required.

Qualifications

• Educational Background: Possess an Associate's Degree in Nursing.

• Work Experience: Bring at least two years of relevant professional experience.

• Certification: Hold a current and valid registration as a Nurse.
