    SUMMARY

    Reporting to the Director CVHP Operations, CVHP Operations, this position is responsible for working closely with health plan member clients, physicians, pharmacists, and physician office staff, via phone, internet, in person and/or through group seminars, and distribution of health education materials to promote wellness. The incumbent is responsible for advising and counseling on the important requirements for a healthy lifestyle, working with clients to identify health risk factors and manage chronic illness, and motivate individuals to realize their optimum health potential. This position also advises and counsels on the important requirements of Medicare STARS, Healthcare Effectiveness Data and Information Set (HEDIS), and Hierarchical Condition Categories (HCC) with provider staff to identify health risk factors and gaps in quality of care. The incumbent will interface with clients using motivational interviewing and reflective listening skills in order to develop an understand of barriers to meeting health related goals, develop a client specific wellness plan, set goals for health improvement, and monitor goal attainment. The incumbent will coordinate services with other health-related departments and organizations for optimal patient care and satisfactory outcomes.

    REQUIREMENTS

    1. Bachelor's degree in Nursing, Health Education, or related field is required. High school diploma or equivalent and current licensure as a Licensed Vocational Nurse (LVN) in the State of California and three (3) years of experience as an LVN may substitute for the degree requirement.

    2. Three (3) years of experience working in health promotion, chronic illness management, or other clinical population health field is required. Experience in an ambulatory care setting is preferred.

    3. Knowledge of specific disease and lifestyle related topics such as smoking cessation, weight management, nutrition, pre/post-natal care, stress reduction and chronic conditions is required.

    4. Proficiency in compilation of statistical reporting, computer skills (spreadsheets, word processing, database), and effective written and verbal communication skills are required.

    5. Demonstrated ability to function independently, strong organizational skills and flexibility are required.

    6. Ability to develop positive working relationships with a wide variety of individuals is required.

    7. Experience in quality and/or data analysis is required.

    8. Bilingual fluency in English and Spanish is required.

    9. Experience with Electronic Health Record preferred.

    10. Certification from an accredited professional wellness coach training program is preferred.

    Pay Range ($27.72 - $38.80)
