As a Community Health Worker (CHW) , you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. Interesting in learning to work with medically complex patients who may be experiencing significant addiction and/or behavioral health conditions is important for this role.
This position is full-time (40 hours/week) Monday - Friday. normal business hours between 8:00am to 5:00pm. It may be necessary, given the business need, to work occasional overtime. This position is a combination of telephonic and field-based with a home-based office. You will work from home when not in the field.
Location: Ocean, Burlington, Mercer, Camden, Gloucester and surrounding cities, NJ
If you are located within near Ocean, Burlington, Mercer, Camden, Gloucester, or other surrounding cities in New Jersey, you will have the flexibility to work remotely* as you take on some tough challenges.
This is a Field Based role with a Home-Based office.
Primary Responsibilities:
Create a positive experience and relationship with the member
Help member set goals and develop a care plan to achieve those goals
Proactively engage the member to manage their own health and healthcare
Support the member to improve their wellbeing by staying out of the hospital, and attend regular visits to their primary physician
As needed, help the member engage with mental health and substance use treatment
Support the member to ensure pick-up of their prescriptions
Provide member education on community resources and benefits
Conduct post discharge activities from hospital facility and support connection to social services
Support member to engage in work or volunteer activities, if desired, and develop stronger social supports through deeper connections with friends, family, and their community
Partner with care team (community, providers, internal staff)
Knowledge and continued learning of community cultures and values

Required Qualifications:
High School Diploma / GED (or higher) OR 4+ years of equivalent community outreach work experience
1+ years of field-based experience
1+ years experience of knowledge of the resources available, culture, and values in the community
Must reside within a commutable distance to Ocean, Burlington, Mercer, Camden, Gloucester and surrounding cities, NJ
Valid Driver's license, current automobile insurance with access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
Ability to travel locally, up to 100 miles round trip and up to 50% of the time
Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and be able to use multiple web applications
Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
Preferred Qualifications:
Associate degree (or higher) in a health-related field
LPN (Licensed Practical Nurse) Licensure or CNA / HHA
Community Health Worker (CHW) Accreditation
Experience working in Managed Care
Knowledge of Medicaid/Medicare population
Soft Skills:
Strong communication and customer service skills both in person and via phone
Ability to work independently and maintain good judgment and accountability
Demonstrated ability to work well with others
Strong organizational and time management skills
Ability to work in a fluid environment
Ability to multi-task and prioritize tasks to meet all deadlines

New Jersey Residents Only: The hourly range for New Jersey residents is $19.47 to $38.08 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
