    Short Description:

    The EMS case manager provides crisis intervention, consultation, referrals, and brief interventions to patients receiving buprenorphine via EMS. The role also completes follow-up with patients to assist in treatment linkages and navigating complex social service applications such as housing applications, disability enrollments, etc.

    Experience Required:

    Completes patient assessments to determine needs


    Provides patients with relevant resources


    Uses brief interventions to refer and link patients to appropriate services


    Provides administrative support and assists in program development


    Uses brief interventions to link patient to appropriate services


    Supports EMS navigator and paramedic teams in addressing SDOH barriers for EMS clients


    As needed accompany team members in conducting community outreach to engage hard to reach clients.

    Education Requirements:

    Bachelor's Degree

    License/Certification Requirements:

    Certified Social Worker or Licensed Social Worker Preferred

    Special Requirements:

    Lived experience and/or experience working in harm reduction or substance use treatment spaces preferred.
