GENERAL DESCRIPTIONPerforms complex (journey-level) social services work. Work involves serving as a liaison between clients and their families in obtaining assistance and rehabilitative treatment. May train others. Works under general supervision with moderate latitude for the use of initiative and independent judgment.
ESSENTIAL DUTIES AND RESPONSIBILITIES- Demonstrate high ethical standards, trustworthiness, and personal integrity. Regular attendance, dependability, and promptness are required to provide quality and consistent services to ECI children and families.
- Interact effectively with all customers, internally as well as externally. Willing to accept coaching and constructive criticism from others while developing effective working relationships with all team members. Support team concepts and cooperate with others to achieve team objectives.
- Complete the intake process for assigned referrals within 7 working days of the referral date 100% of the time or document all barriers encountered that prevent meeting this timeline. The intake process includes conducting the phone interview to establish concerns and sending the parent information by mail or email to the family.
- Schedule children for evaluation or IFSP (medical qualifiers) with appropriate evaluation teams in order to meet the 45-day timeline 100% of the time or document all barriers encountered that prevent meeting this timeline.
- Discontinue referrals that are parent declined or could not contact-completing all documentation to support efforts to contact.
- Complete developmental screens as needed.
- Evaluation/Assessment and Treatment planning - Work as a strong and vital part of a transdisciplinary team to provide essential and in-depth input on the development of referred children 0-3 years. Use approved screening and evaluation tools, observation of developmental skills, and interview techniques with the family, to provide thorough and accurate assessment/evaluation and child outcome ratings. Develop, review, and/or update IFSP's along with the child's transdisciplinary team. Develop appropriate outcomes that address the child's needs and reflect the parent's priorities and also include consideration of natural environments and child and family routines.
- Resolve status of assigned cases within 45 days of referral 100% of the time or document all efforts to overcome any barriers that prohibited compliance with this requirement.
- Opens all charts of newly enrolled children within 5 working days of enrollment 100% of the time. Provide enroller email to all team members within 3 working days of the IFSP.
- Completes all required referrals and request for physician's authorization within 5 days of enrollment.
- Provide all required documentation for newly enrolled children to the child's service providers by Friday of the week that the child is enrolled 100% of the time, with exception of children enrolled on Fridays, this documentation will be given to appropriate service provides by the following Monday.
- Case Management - Provide a single point of contact for a child's family in order to coordinate services and to facilitate accessing needed services. These services include but are not limited to ECI services, community resources, medical services, and transition from ECI. Assess and monitor child and family needs as documented in the IFSP by conducting an IFSP monitoring contact for each child on your caseload a minimum of one time per month 100% of the time or provide adequate documentation to show all efforts to overcome barriers that prohibited this contact. Follow up on referrals to ensure the child receives appropriate services. Keep team members aware of child and family needs as necessary. Communicate regularly with team members as part of ongoing needs assessment. Thoroughly and accurately explain the FCS system to families and correctly complete the required FCS paperwork as needed or mandated by TAC.
- Provide service coordination activities for all children on staff's caseload as needed or mandated by TAC. Carry a caseload of 55-65 Case Management only children.
- Ensure that all transition requirements, to include development of transition steps and strategies, transition conference and referral for special education services, are met by the established timelines 100% of the time. Document efforts to overcome any barriers to transition being completed in a timely manner.
- Complete required 6 month reviews and annual eligibility determinations/IFSP reviews by the due dates 100%of the time. Document any barriers that prohibit meeting this requirement.
- Clearly, legibly and thoroughly document all services provided to include ongoing progress notes, case management notes, consultation notes, correspondence, and IFSP's.
- Turn in documentation and other paperwork to include employee time sheets, expense voucher, and other employee and ECI child documentation by the due dates 100% of the time.
- Write the majority of progress notes in the home with input from the family. All notes must contain required elements.
- Feedback from 95% of families served is positive. The feedback may be from parent satisfaction questionnaires sent out by the program, from phone calls or other contacts made by supervisors to families, and/or from family-initiated contacts with the program or the state office.
- Participate in the Random Moment Time Study as required, including the required training.
- Professional Development - This function requires the employee to successfully complete training and continuing education as required by professional standards and the Center in a timely manner. The employee will engage in learning opportunities and activities that enhance and improve their service delivery to children and families.
- Support and mentor other ECI staff with processes and policies as needed. Contribute toward the training of parents and/or community members as appropriate.
- Support program events such as Child Find activities and other special events.
- Accept assignment of services to be provided in a language other than English as determined by the primary language of the referred/enrolled children and their routine caregivers. Provide support in the form of translation and interpretation to other team members serving these same children and families.
- Meeting this job requirement makes the staff member eligible for the bilingual stipend according to the PPM policy 205:2Must stay compliant with all required training.
- Frequent daily driving within the catchment area is required.
GENERAL QUALIFICATIONSExperience/Education/Licensing/Certification- Bachelors degree from an accredited college/university required
- Dependent on degree emphasis, hours must include at least 9, 12, or 15 hours relevant to Early Childhood Intervention - transcript required
- Prefer experience with young children
