Schedule: Part-Time; 24 hours/week

Shift: 3 8-hour shifts between 8am-4:30pm or 8:30am-5pm; may include weekends

Location: AdventHealth Palm Coast - 60 Memorial Parkway, Palm Coast, FL 32164

The role you'll contribute:

Responsible for continuity of care across the continuum with emphasis on quality patient outcomes and cost-effectiveness through decreased length of stay, and control of resource utilization. Provides Patient/family instruction, scheduled physician appointments, any additional discharge collaboration/coordination in the hospital's EMR and coordinates activities according to patient's need.

· Follows established criteria for utilization of services to ensure denials are avoided Utilizes nursing data base to initiate discharge planning by procuring appropriate level of care at discharge, i.e. self [1] care, nursing facility, home health, physical / psych facilities Acts as liaison with physician to assure appropriate and complete documentation Prioritizes activities and completes work assignments on a timely basis while functioning with minimal supervision

· Assists physicians by providing information, suggestions, alternatives, and timely referrals for discharge planning

· Provides continuity of care by coordinating the flow of information between hospital departments and outside organization.

Qualifications

The expertise and experiences you'll need to succeed:

• High School Grad or Equivalent

· 3 years experience

· State of Florida LPN License
