    How you make a difference

    The Continuity of Care Specialist plays a pivotal role in ensuring smooth transitions for individuals by coordinating and implementing discharge planning with care and precision. This position fosters strong communication between facility staff and community resources, ensuring seamless continuity of care. By maintaining accountability for services provided, the Continuity of Care Specialist helps deliver a positive and supportive experience for individuals transitioning out of care. If you are passionate about making a meaningful impact on individuals' lives and ensuring quality care continues beyond the facility, this is the role for you!

    Key Responsibilities

    • Discharge Planning : Responsible for creating and implementing discharge plans, ensuring adherence to policies and procedures.

    • Stakeholder Relationship Management : Develop and maintain relationships with internal and external stakeholders, including treatment teams and community agencies serving individuals with mental health or serious medical conditions.

    • Financial Assistance and Benefits : Facilitate applications for financial assistance and help individuals resume benefits as part of the discharge process.

    • Life Skills and Support Activities : Provide individual and group activities to enhance coping, life skills, and support for individuals upon release.

    • Continuity of Care Coordination : Coordinate medical and medication needs upon release, assist with housing, and make appointments with mental health agencies to ensure continuity of care.

    Qualifications & Requirements

    Education:

    • Required: High School diploma or equivalent.

    • Preferred: Bachelor's degree in social work or closely related field from an accredited college or university.

    Experience:

    • Preferred: Prior experience developing transition plans for individuals with mental illness.
