    The Complex Care Navigator (CCN) helps patients/families/caregivers, facing serious illness and/or complex care needs, navigate the health care systems and collaborates with the Complex Care program team to incorporate patient goals with medical reality. The goal for the CCN is to form strong relationships with patients/families/caregivers, to bridge the gap between medical knowledge, the healthcare system, and the patient's lived experience and worldview. This is a non-clinical role with strengths in relationship building and communication, high emotional intelligence or "EQ" is required.

    This position is full-time, Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 7:30 am - 5:00 pm PST. It may be necessary, given the business need, to work occasional overtime. Employees are required to work some days onsite and some days from home.

    We offer 4 weeks of on-the-job training. The hours of the training will be aligned with your schedule.

    If you are within commutable distance to the office at 904 7th Avenue Seattle, WA, you will have the flexibility to work from home and the office in this hybrid role* as you take on some tough challenges.

    Primary Responsibilities:

    • Other duties may be assigned, but those responsibilities listed below constitute the bulk of those associated with this role.

    • Receive referrals from primary care clinicians and enrolls patients with serious illness into complex care programs, explains the program, discuss palliative care as appropriate, and acts as patient's key point of contact.

    • Contact patients, families, and caregivers to arrange visits as needed by phone, office, or home visit.

    • Collaborates as "substitute" high functioning family member for the patient, gaining knowledge of patient's approach to care, resources, medical literacy, and helping them to navigate the medical system.

    • Works in highly interdisciplinary mode, is a conduit between provider / case managers / etc. and patient / family.

    • Track and monitor work using the electronic medical record.

    • Collaborate with other clinical staff and allied health professionals including social workers, palliative care educators, nurse care managers, physicians, making contributions as necessary as one of the members of interdisciplinary teams.

    • Run daily reports for daily discharges and work under RN Case Manager for delegation of outbound calls.

    Required Qualifications:

    • High School Diploma / GED OR equivalent work experience

    • Active Medical Assistant Certification

    • Must be 18 years of age OR older

    • 2+ years of customer service experience

    • 1+ years of healthcare experience working in physician's office OR medical clinic setting

    • Experience with Microsoft Word, Microsoft Excel and Microsoft Outlook

    • Ability to work between the hours of 7:30 AM - 5:00 PM PST from Monday - Friday including the flexibility to work occasional overtime based on business need

    Preferred Qualifications:

    • Experience with working within the health care Industry and with health care insurance

    • Experience with working in a hospital

    • A clerical OR administrative support background

    • Knowledge of ICD-10 and CPT codes

    • Experience with working in a call center

    Telecommuting Requirements:

    • Reside within commutable distance to the office at 904 7th Avenue Seattle, WA

    • Ability to keep all company sensitive documents secure (if applicable)

    • Required to have a dedicated work area established that is separated from other living areas and provides information privacy

    • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

    The hourly range for this role is $19.86 to $38.85 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
