    Essential Duties and Responsibilities

    • Serves as the primary care coordinator between high risk patients with serious mental illness, the behavioral health providers, dentists, hospital-based service providers, financial assistance specialist, residential/community/behavioral health case managers, peer specialists, visiting nursing agencies, community health workers, the Project HOME Health Services nursing/medical team and/or other specialists.
    • Manages the prior authorizations, scheduling and administration of antipsychotic injections for all patients at SKWC.
    • Helps with care management and scheduling for patients with serious mental illness living in Project Home Residences in coordination with the Mobile Nursing Team and psychiatric care providers.
    • Demonstrates excellence in clinical care.
    • Educates and counsels patients and their families about issues related to serious mental illness.
    • Demonstrates excellence in care management by coordinating care with intensive emphasis during care transitions.
    • Participate in the Quality and Risk Management Committee as designated.
    • Develop and maintain activities to achieve Patient Centered Medical Home as required.
    • Assists with clinical procedures.
    • Documents clinical and care management activity in NextGen.
    • Provides nursing assessments and triage to walk-in patients as well as handles in-coming ?sick? calls from high risk patients per the Stephen Klein Wellness Center triage protocol.

    Minimum Qualifications

    • Bachelor of Science in Nursing (BSN)
    • 3 years of prior experience as a nurse care manager
    • Active RN license in Pennsylvania
    • BLS certification

    Preferred Qualifications

    • One or more years' experience working as an RN with the target population.
    • Prior experience delivering nurse care management services to patients with complex care concerns
    • Public Health degree a plus
    • Knowledge and prior practice of trauma informed care
    • Proficiency in Microsoft Outlook
    • Prior experience as a Nurse Care Manager in a primary care setting.
    • Prior experience delivering services to patients who have experienced homelessness, mental illness, and/or substance dependence, often complicated by histories of trauma.
    • Strong computer, organizational and administrative skills.
    • EHR experience

    • Minimal lifting, 10 pounds or less
