Job Summary and Qualifications
The Manager, Community Engagement will work with the MHP Care Coordination team to ensure that team members are trained to help patients access the resources and services they need. This position is responsible development and continuity of collaborative relationships with community organizations that offer resources for our patients. Additionally, this position will act as a liaison between MHP, patients, healthcare providers, and various groups and services in the community. As part of MHP team, the specialist will strive build patient centered care, bridge community strengths, and integrate partners to improve patient health and health equity.
What qualifications you will need:
Required Education: Bachelor degree with 5 years of experience in health related field
Preferred Education: Master degree in public health, health care administration of other healthcare related field.
Preferred License: Social work, Nursing, maintains nationally recognized certification in Case Management, such as ACM, CCM or ANA certification) If RN, must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing. eNLC (Enhanced Nurse License Compact) NCBON, LCSWA.
Required Experience: Any combination of 3 years health/social services experience and/or education. Verifiable good driving record. Ability to flourish in a team system that supports equity and inclusion.
Preferred Experience: One year of experience in ambulatory health care preferred. Working with high-risk, vulnerable, or uninsured population. Understanding and sensitivity regarding challenges facing low-income communities. Entry level proficiency in another language such as Spanish or Russian preferred. Computer proficiency – electronic health record experience preferred.
