As a Community Care Registered Nurse (RN), you'll provide both clinical and psychosocial care to individuals experiencing homelessness in community settings. Primary responsibilities include:
Providing care to individuals experiencing homelessness at locations such as encampments, interim housing/shelter sites, community health and resource fairs, and on the streets.
Conducting patient assessments and documenting data through interviews, reviews of medical records, and analysis of diagnostic test results.
Collaborating with providers, social workers, and other health professionals to develop effective care plans.
Sharing responsibility for driving the Community Care Van to various service locations.
Additional Schedule Information
Shifts may start as early as 7:00 a.m. or as late as 1:30 p.m., with typical shift hours as follows:
07:00 a.m. - 03:30 p.m.
08:00 a.m. - 04:30 p.m.
09:00 a.m. - 05:30 p.m.
10:00 a.m. - 06:30 p.m.
11:00 a.m. - 07:30 p.m.
01:30 p.m. - 10:00 p.m.
Because we provide patient care seven days a week, all RNs work at least two weekend shifts per month. Schedules are provided in advance.
Salary Range: $59.46/hour - $88.39/hour
sharing the responsibility for driving the Community Care Van.
