    PeaceHealth is seeking a Clinical Pharmacy Specialist Ambulatory for a Full Time, 1.00 FTE, Day position.

    The salary range for this job opening at PeaceHealth is $67.93 - $101.89.

    The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

    Job Summary

    Position is responsible for collaborating with PeaceHealth Medical Group clinicians and staff to provide coordinated, comprehensive, high-quality care to patients, focusing on medication management.

    Details of the Position

    • Embraces and champions all ambulatory based services inclusive but not limited to primary Care, specialty care, refill clinics, PA support, pain management, annual wellness visits, anticoagulation services, provider education, comprehensive medication management & medication safety.

    • Provides evidence-based medication management services to patients utilizing a Collaborative Drug Therapy Agreement.

    • Implements and monitors clinical protocols and guidelines and enhances the delivery of evidence-based medicine for services outside of a Collaborative Drug Therapy Agreement.

    • Serves on and provides consultation to PeaceHealth Medical Group committees as requested.

    • Provides drug-therapy related education and training to team members as well as patients.

    • Documents clinical activities and outcomes.

    • Participates in initiatives to enhance pharmaceutical care and medication safety.

    • Detects, monitors and reports adverse drug events and medication errors.

    • Oversees orientation, training and mentors pharmacy students, pharmacy residents, and pharmacy technicians.

    • Performs other duties as assigned.

    What You Bring

    • Bachelor of Science: Pharmacy through an APCE-accredited school of pharmacy (Required) and

    • Doctor of Pharmacy (Preferred) or

    • Preferred: . BSc Pharmacy or BSc Pharmacy Board of Pharmacy Specialties Certification in specialties related to ambulatory care (BCACP, BCGP, etc.) or successful completion of a Post-Graduate Year Two (PGY2) residency or successful completion of a Post-Graduate Year Two (PGY2) residency

    • Minimum of 1 year Pharmacy experience in ambulatory care (Required) and

    • Comprehensive knowledge of evidence-based clinical pharmacy practice in intended area(s) of ambulatory care specialty, including related equipment, techniques, practices and procedures (Required) and

    • Minimum of 3 years Advanced pharmacotherapy specialist practice in an ambulatory clinical pharmacy setting with primary focus on direct patient care, medication management, consulting and teaching (Preferred)

    • Multi-State Registered Pharmacist in the state(s) in which you will be working (Required) and

    • Required within 2 years: Board of Pharmacy Specialties in specialties related to ambulatory care (BCACP, BCGP, etc.) and

    • Required within 6 months: Anticoagulation Certification ASHP Anticoagulation Certificate OR University of South Indiana Anticoagulation Therapy Management Program Certificate and

    • Required within 90 Days: Basic Life Support Basic Life Support (BLS)

    Skills

    • Ability to meet payor credentialing requirements for pharmacist billing of clinic provided services. (Required)

    • Ability to practice under a Collaborative Drug Therapy Agreement on file with respective state of network location. (Required)

    • Ability to implement, document, and monitor, with health care team, patient anticoagulation management for achievement of desired therapeutic outcomes. (Required)

    • Proficient use of computers including MS Office applications. Experience with computerized pharmacy systems. (Required)

    • Excellent communication skills both verbal and written. Excellent presentation skills. (Required)

    • Ability to work collaboratively as a member of an interdisciplinary team, and establish and maintain effective working relationships with other healthcare professionals, practitioners, patients and families. (Required)

    • Ability to precept and obtain certification where needed to oversee pharmacy students/residents (Required)

    • Ability to effectively support, coach, educate, mentor and lead staff. (Required)

    • Strong leadership and organizational skills. (Required)

    Working Conditions

    • Ability to move around area with occasional sitting.

    • Occasional squatting/ kneeling/ bending.

    • Fine motor skills to be able to grasp to perform precise procedures frequently.

    • Frequent reaching.

    • Lift/Carry: Occasionally up to 30 lbs.

    • Push/Pull: Occasionally up to 14 lbs. force.

    • Exposure to biohazard, body fluids and airborne particles.

    • Must be able to complete tasks in a noisy environment.

    • Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).

    • Ability to communicate and exchange accurate information.
