    The Clinical Liaison serves as the account owner of the assigned hospitals, providing education and information on the organization's services, and acts as a liaison between the assigned hospitals and the hospice team. The Liaison makes inpatient visits to hospice patients in hospitals and provides education to providers. The Clinical Liaison is responsible for building and developing relationships with physicians, case managers, social workers, and other key decision makers throughout the assigned hospitals to support the mission of the organization.

    Essential Duties & Responsibilities:

    • The Clinical Liaison will present professionally, have good problem-solving capabilities, have excellent interpersonal communication, and have excellent customer service skills. This role serves as an organizational ambassador within the assigned hospitals.
    • Record the results of the nursing assessment into organization's EMR and hospitals' preferred mode, if indicated
    • Abide by and demonstrate the organization's Mission, Vision, and Values through both behavior and job performance on a day-to-day basis
    • Develop and maintain contact with key physicians, hospital staff to include case managers, social workers, and discharge planners
    • Promote exemplary customer service to physicians, hospital personnel, patient and family, at all times
    • Collaborate with other health care providers in determining the appropriate patient and family-centered plan of care for the patient and the family and ensure the implementation of the interdisciplinary plan of care
    • Make inpatient visits to hospice patients in hospitals
    • Provide for the maintenance of safe and effective nursing care, whether rendered directly or indirectly
    • Perform other but similar duties as assigned
    • Participates in Quality Assurance and Performance Improvement activities
    • Serves as end-of-life resource to hospital providers, nursing staff, social workers, discharge planners, case managers, chaplains, etc as well as to patients and/or family members.
    • Coordinates transfer of care for hospital referrals for routine and General Inpatient admissions from hospital to long term care facilities, home or Patient Care Units

    Education:

    • Bachelor's degree in nursing.

    Experience: 2+ years in field of nursing, hospice experience preferred.
