Why This Role is Important to Us:
The Clinical Director, Hospital to Home, is responsible for leading CCA's engagement with acute care hospitals and developing strategies and partnerships to support CCA members through transitions across care settings. The position provides supervision and leadership to a team of facility-based providers (medical and behavioral health) as well as community health workers, and is additionally expected to perform member-facing work in acute care settings.
Key goals include effectively engaging and partnering with stakeholders at CCA's highest-volume hospitals (including but not limited to Baystate Medical Center, Boston Medical Center, MGB Salem Hospital, and Beth Israel Deaconess Medical Center), streamlining and aligning processes both internally (across CCA departments) and externally (with hospitals and network providers), improving member experience through care transitions, promoting the appropriate utilization of clincial services, and reducing unnecessary hospital admissions and readmissions.

What You'll Be Doing:
Understand the complex physical, behavioral health and social needs of CCA members to leverage the appropriate resources to reduce unnecessary hospital admissions in members presenting to ER settings
Clinically supervise Advanced Practice Clinicians and/or Registered Nurses
Lead and manage an interdisciplinary team
Collaborate closely with Behavioral Health Clinicians and Community Health Workers
Support clinical best practices and lead the development of practice guidelines
Develop clinical quality metrics and performance improvement initiatives
Interface with clinicians in the community (e.g. PCPs, specialists, and hospital-based clinicians) to promote an understanding of CCA's unique model and facilitate collaboration
Lead monitoring, evaluation, and quality improvement activities
Participate in clinical operational meetings as requested by supervisor and/or other managers at CCA
Document all work in a timely and accurate manner
Lead clinical team meetings and participate in social & teambuilding events
Conduct educational and training activities that promote appropriate and effective patient care
Participate in the planning, implementation and coordination of health education and counseling of the patient
Participate in orienting and training new employees
Participate in case review and quality improvement projects
Maintain professional licensing, credentialing, and CME
Attends external meetings and activities as a representative of the organization as requested
Working Conditions:
Standard working conditions.
Standard hours are Monday - Friday 8:30 a.m. - 5 p.m
What We're Looking For:
Qualifications:
DNP or NP with degree from an accredited institution.
Active Massachusetts Certified Nurse Practitioner license, in good standing.
7+ years of clinical experience
Experience in emergency room and/or hospital settings
Interest and experience in serving underserved and disability populations
Experience with quality improvement, monitoring and evaluation, health systems strengthening, innovation and training
Proven skills, knowledge base and judgment necessary for independent clinical decision-making
Excellent organizational, time-management and problem-solving skills
Ability to function effectively as part of a multi-disciplinary team
Excellent oral and written communication skills
Strong interpersonal skills
Commitment to social justice in medicine and an understanding of healthcare reform
Effective teaching and mentorship skills
Familiar with a variety of the field's concepts, practices, and procedures
Ability to lead and direct the work of others
A wide degree of creativity and latitude is required
Able to support teams with utilization of telehealth technologies (video, chat, etc.), when appropriate, as an approved modality for a variety of clinical care and care management services
Seeks to maintain a constructive work environment and maintains effective communication with other employees and managers
