  • Overview

    The Chemical Dependency counselor Case Manager is responsible to perform services encompassed in the essential activities

    of case management, which are, assessment, planning, implementation, coordination, monitoring, and evaluation for assigned

    population. Direct client, family contact would include activities in the areas of coordination and service delivery, physical and

    psychological factors, benefit systems and cost benefit analysis, case management concepts, and community resources. The

    Case Manage provides services across a continuum of care that addresses the ongoing needs of the individual being served by

    the case management process. These services are provided in an environment that involves multiple settings, interact with all

    relevant component of the client's health care system, and deal with the individual's broad spectrum of needs.

    Responsibilities Job Description

    *Coordinates the delivery of service.

    • Understands legal and ethical issues pertaining to confidentiality.

    • Understands medical terminology.

    • Understands restrictions on the release of confidential information.

    • Knows how to obtain an accurate history.

    • Establishes treatment goals that meet the client's health care needs and the referral source's requirements.

    • Establishes working relationships with referral sources.

    • Assesses clinical information to develop treatment plans.

    • Communicates case objectives to those who need to know them.

    *Identifies physical and psychological factors that can affect care and outcomes of care.

    • Identifies cases with potential for high-risk complications.

    • Acts as an advocate for an individual's health care needs.

    • Understands methods for assessing an individual's present level of physical/mental impairment.

    • Understands the physical characteristics of illnesses.

    • Understands the psychological characteristics of disabling conditions.

    • Understands the psychological characteristics of illnesses.

    • Assists individuals with the development of short and long-term health goals.

    • Understands the psychological characteristics of wellness.

    *Understands benefit systems available and performs cost-benefit analysis.

    • Evaluates the quality of necessary medical services.

    • Understands requirements for prior approval by payer.

    • Identifies cases that would benefit from alternative care.

    • Evaluates necessary medical services for cost containment.

    • Analyzes individual's care plan for appropriateness.

    • Acquires data necessary to determine the cost of care.

    • Understands home health and community resources.

    • Understands health care delivery systems.

    *Incorporates Case Management concepts into all phases of work. · Understands the role of the case manager. · Documents case management activities. · Applies problem-solving techniques to the case management process. · Understands case management philosophy and principles. · Knows how to evaluate the effectiveness of case management. · Understands planning and goal development techniques. · Understands liability issues for case management activities. · Develops case management plans that address the individual's needs.

    *Demonstrates competence in developing a clinically relevant transition/discharge plan.

    • Understands interviewing techniques.

    • Incorporates the patient, family and interdisciplinary team as well as community contacts into developing the discharge

    plan.

    • Knows how to explain services and available resources (including limitations) to individuals.

    • Knows how to establish a client's support system.

    • Understands assistive devices needed for individual.

    • Understands the Americans with Disabilities Act.

    • Understands federal legislation affecting individuals with disabilities.

    • Understands the client's need for vocational services.

    • Knowledge of Community resources and treatment facilities.

    *Communicates effectively and respectfully with others.

    • Communicates frequently and effectively with the interdisciplinary team and outside involved parties to ensure continuity

    and coordination of care.

    *Demonstrates clinical competence in conducting group

    *Provide Rule 21 assessments and CD evaluations as needed.

    Qualifications

    Education

    Minimum Education

    Bachelor's

    Human Service's related field

    Minimum License/Certification/Registration

    Licensed Alcohol and Drug Counselor (LADC)

    Experience

    Minimum Experience

    5 - 7 years full time case management experience encompassing work with all member age ranges and multiple

    payors

    Preferred Experience

    Utilization and quality management experience

    Prefer work experience working with teens with Substance Use Disorder and dual diagnosis

    Prefer work experience working with families with teens

    Additional Requirements

    Experience and skills reflect the six essential activities of case management and the five core areas of case management and

    associated work activities. Demonstrated skills in using a personal computer.

    • No felony conviction or have been incarcerated as a result of a felony conviction in the last three years

    • Have not had a chemical use problem during the last two years.
