  • The Preadmissions Department is looking for a Chart Facilitator to join their growing team. Applicants should be detail oriented with an ability to work autonomously. This role will require good communication and computer skills and must be comfortable reviewing 50 patient charts per day at minimum. This is a flexible position with ability to work Monday - Friday with work from home flexibility on most days.

    Purpose:

    Responsible to maintain the accuracy of both the physician delinquency process and the master files and facilitate the timely completion of patient records and updated physician information. Ensures the integrity of the data in our Legal Health Record is accurate, complete and available to support patient care, JCAHO and DOH requirements. Provides orientation to the chart completion process, accessing the legal health record through e-HIM repository, and the Chart Deficiency procedure.

    Responsibilities:

    • Know and follow all organizational/departmental policies/procedures.

    • Audits compliance to meet JCAHO and DOH requirements.

    • Maintains the electronic Death Registry and the Gift of Life referrals, coordinating with physician staff and funeral directors to obtain completion of death certificates.

    • Follow up on missing discharged records to ensure records are received for processing.

    • Reports any trends to management.

    • Prepares, scans and validates records while performing legibility and integrity analysis; scans loose papers into the electronic health record.

    • Actively practice behaviors associated with SH Service Excellence expectations.

    • Completes scanning, QA and analysis processes in established time frames to support department goals.

    • Provides orientation to the physicians and physician assistants and mid-wives, acclimating them to the chart completing process, accessing the legal health record through e-HIM repository, and the chart deficiency procedure.

    • Updates the master files (NPI Process), merge medical records, monitor, correct and follow up on printed reports.

    • Copy charts as needed for transfers and/or release of information requests.

    • Performs all functions related to the delinquency list, physician completeness process, records over 30 days requirement, and re-analysis.

    • Assume responsibility for personal development and education.

    • High School Diploma or equivalent required.

    • Electronic Health Record or Health Information Management course work preferred. Licensure, Certifications, and Clearances:
