    Description

    SUMMARY

    Under the supervision of the Director of Social Services, provides direct case management services to clients.

    ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and responsibilities may be assigned.

    • Conducts an initial intake meeting within 24 hours of the client's arrival to the Sanctuary Program.

    • Determines clients' needs by completing intake interviews; and ensures clients are orientated to the program guidelines.

    • Coordinate referrals for healthcare, mental health, and educational services.

    • Enhances the program, by maintaining community relationships with all providers.

    • Maintain and prepare clients' records, by reviewing case notes.

    Qualifications

    REQUIRED EDUCATION

    • High School Diploma / GED Equivalent

    • Bachelor's degree in social work or human services field. preferred

    • Bilingual in Spanish, preferred

    REQUIRED EXPERIENCE

    • Minimum two years in a behavioral health/substance use disorder treatment program.

    WORK ENVIRONMENT

    • This job operates in an indoor office environment. This role routinely uses standard office equipment such as computers, phones, and photocopiers.
