    SUMMARY

    Family Service League is seeking a full-time Case Manager for the Adult Care Management State Funded program. The Case Manager meets the intensive needs of the population while meeting visit requirements as stipulated by OMH for the Adult Care Management State Funded program. Clients include persons diagnosed with serious mental illnesses and possible additional medical and substance use diagnoses.

    RESPONSIBILITIES

    • The Case Manager will maintain a positive, working rapport with Suffolk County Adult SPOA.
    • Employ a culturally sensitive and proactive outreach approach to potential members.
    • Manage a caseload of 25-35 clients and meet their intensive needs.
    • The Case Manager will meet minimum visit requirements based on client's determined acuity level and needs.
    • Speak with mental health, medical, and substance abuse providers in addition to hospitals, jails, and housing/shelter providers if necessary.
    • The Case Manager will form and maintain a positive and professional relationship with both internal and external providers in order to coordinate care and obtain needed information.
    • Complete all required documentation in a timely and accurate manner.
    • Report all incidents to the program management staff.
    • Follow all agency and program mandates as set by FSL, Department of Health, and Suffolk County Division of Mental Hygiene.
    • All other duties as assigned.

    QUALIFICATIONS

    Bachelor’s Degree in a health-related field required.

    Minimum of 2 years of experience providing direct services to people with serious mental illness required.

    Proficiency in Microsoft Office required; experience with an EHR is preferred.

    Strong assessment, engagement, organizational and time management skills.

    Excellent verbal and written communication skills are required.

    Valid and clean NYS Driver’s License required.

    PHYSICAL REQUIREMENTS

    This position often requires sitting and working at a computer for extended periods of time, traveling to different FSL offices, community meetings, home visits, etc.

    Compensation details: 40000-45000 Yearly Salary
