    Position Overview

    Come be a part of our team in the role of Universal Care Manager for our Community First Choice (CFC) Program, a crucial position that significantly impacts the lives we touch. As a symbol of compassion and commitment, you will play a key role in organizing and overseeing personalized services, guaranteeing that our clients enjoy a life of independence and dignity within the comfort of their communities.

    Key Responsibilities

    • Perform detailed assessments to evaluate the needs and preferences of our participants.

    • Craft and execute tailored care plans in collaboration with participants, their families, and other caregivers.

    • Oversee the provision of services, guaranteeing alignment with the goals and needs of our clients.

    • Offer continuous support and advocacy for our participants, proactively addressing concerns.

    • Keep meticulous records of all services and interactions, ensuring information is current and accurate.

    • Work in partnership with community resources and service providers to fortify the support network for our clients.

    • Engage in ongoing educational and professional development opportunities to remain at the forefront of care management practices.

    Qualifications

    • Bachelor’s degree in Social Work, Nursing, or a related discipline.

    • A minimum of 2 years experience in care management or a similar role.

    • Deep understanding of the unique needs and challenges confronting older adults and individuals with disabilities.

    • Exceptional skills in communication, organization, and problem-solving.

    • Self-motivated with the ability to excel independently and as a team member.

    • Competency with electronic health records and relevant technological tools.
