    Job Description:

    The Care Management Associate supports the RN Care Coordinator utilizing excellent organizational and communication skills and showing a personal commitment to providing compassionate care.

    Education Qualifications:

    Key Responsibilities

    • Provides communication with patients, care teams, and other stakeholders to ensure that all relevant information is available to provide the best possible quality of care

    • Assists the RN Care Coordinator with scheduling appointments, coordinating care, providing follow-up services to patients, submits data to health plans, and any other duties as assigned

    • Tracks and maintains patient records and insurance information

    • Provides administrative support including data entry, documentation in the Electronic Medical Record, and other duties as required

    • Collaborates with other departments to ensure the efficient delivery of care while demonstrating a strong commitment to providing compassionate and quality care

    Required Qualifications

    • Medical Secretary, Health Unit Coordinator, or similar program

    • Minimum of 2 years' experience in a medical setting if no education meets the Education Requirements listed
